ESSEX HIGH SCHOOL COACHES REQUEST FOR ESSEX BOOSTERS DONATION Form

Essex High School  head coaches should use this form to submit formal requests for a donation from the Essex Boosters to support your EHS athletic program/team. 

Please encourage your teams & parents/caregivers to volunteer at Essex Boosters Concession Stands!

Please note the following:

• Requests can only be submitted by the Head Coach of the Essex High School athletic program/team
• Requests can only be submitted immediately before the start of the season OR during the season
• Include as much information as possible with your request including screenshots/attachments where relevant
• Donations will be considered for items such as equipment, uniforms, registration fees, transportation, and other expenses directly related to the team
• Donations will not be considered for team gifts, awards, or meals
• The Essex Boosters Executive Board will review each request and provide a response within 2 weeks from submission
• Amounts approved may be less than the requested amount
